    Way to block a stolen / lost cellular phone:

    Follow and complete the following steps:

  • Bring photocopy of 2 valid ID's (company,school,Passport,TIN/SSS/GSIS, Driver's or PRC Lic.)
  • Bring Proof of Ownership(e.g.) purchase receipt or barcode sticker
  • In the absence of proof of ownership, bring Police Report
  • Fill up AFFIDAVIT OF LOSS form and duly notarize
