ID CARDS
1. DO NOT LET YOUR ID CARD EXPIRE. ID Cards may be renewed up to 90 days prior to expiration. Please keep in mind the DEERS workstation, or computer, frequently crashes or does not function properly. Always call before making a trip to the DEERS office to check system status, to ask any questions, or to have any problems addressed. At age 65, both retiree and dependents must obtain a new ID card for proof of TRICARE for Life eligibility and to update DEERS. To obtain an ID card within the Medford area, go to the Oregon Army National Guard Armory, HWY 99 and Stewart Avenue in Medford, OR. Monday thru Thursday 9am - 4pm and Friday 9am - 1pm. Please call 541-245-4328 before going to ensure the DEERS equipment is functioning.

2. To obtain an ID Card you must have the following documents:
  1. Military Retiree:
  2. Old ID Card or Drivers License, Social Security Card, Last DD214 or Retirement Orders, and if you are over the age of 65 or disabled, your Medicare Card.
  3. Spouse of Military Retiree:
  4. Sponsor must be present and possess a valid ID Card. You'll also need your Marriage Certificate, Social Security Card, and if you are over age 65 or disabled, your Medicare Card.
  5. Children of Military Retiree:
  6. Sponsor must be present and possess a valid ID Card. You'll also need your child's Birth Certificate, and Social Security Card.
  7. Widow of Military Retiree:
  8. You must have your old ID Card, Drivers License, Social Security Card, sponsor's last DD214 or Retirement Orders, sponsor's Death Certiifcate, Marriage Certificate, and if you are over the age of 65 or disabled, your Medicare Card.

4. Obtaining your DD-214 via the Web The National Personnel Records Center (NPRC) has provided the following website for veterans to gain access to their DD-214s online: http://vetrecs.archives.gov/
This may be helpful when a veteran needs a copy of his DD-214 for employment purposes. NPRC is working to make it easier for veterans with computers and internet access to obtain copies of documents from their military files. Military veterans and the next of kin of deceased former military members may now use a new online military personnel records system to request documents. Other individuals with a need for documents must still complete the Standard Form 180, which can be downloaded from the online website. Because the requester will be asked to supply all information essential for NPRC to process the request, delays that normally occur when NPRC has to ask veterans for additional information will be minimized. The new web-based application was designed to provide better service on these requests by eliminating the record center's mailroom processing time.

INSTRUCTIONS FOR ORDERING EYEGLASSES FOR RETIRED MILITARY PERSONNEL

If you are retired military, and eligible for other medical services, you are authorized to receive one pair of Standard Issue spectacles per year. The Navy Optical Command (all services may request glasses) website and catalog to view Standard Issue Frames may be viewed on the Internet at: http://nostra.norfolk.navy.mil.

- S-9/male
- S-8/female
- half-eye or
- S91A or S91B (authorized only if patient requires oversize frames)

If you are less than 50 miles from a military health clinic with optometry services, you may take your prescription for spectacles to them so that they may measure you for proper fit. They will order your spectacles from the appropriate facility. If you are more that 50 miles from a military health clinic with optometry services, you may follow these instructions to order your glasses.

Print the form DD771 from the NOSTRA web site (http://nostra.norfolk.navy.mil/library/DD771_2006.pdf).

Please fill out the top section with your name, retiree, and last 4, along with your shipping and contact information. About halfway down the form, please indicate which standard issue frame you desire.

Take this partially completed form with you to your eye exam. Ensure that the examining facility writes the prescription, and includes the pupillary distance (PD) on the form.

If you currently have Standard Issue frames please provide the frame size and temple length. (found inside the frame)

There must be a DD771 for each request for eyewear.

- One copy of the completed DD771 form,
- A copy of your military ID card.
- Your mailing address and a daytime phone number and email address if available.
