Membership Forms Download
Please read our Statement of Faith
You can download the entire Membership Form (including the Statement of Faith) from this page. These forms are used for establishing your chapter membership.
If you are a new member, you will need to download, print, and complete both documents. It is necessary for these forms to accompany your dues payment, to be sent to our Chapter Treasurer at the address printed at the top of the Membership Form. Click here for Membership Forms.
If you are an existing member, you will not need to download these forms, as you will be provided with a member information sheet at renewal time, containing the information that we already have on file from intial registration or last update. You will use that information sheet to correct whatever data requires updating and submit that with your renewal dues. A new statement of faith is not required at renewal, as we have your original on file.
If you are a former member who wants to rejoin, contact Cathi before proceeding. It is entirely possible that your last Membership Renewal form can be retrieved from our records and you will not need to download these forms. As in the case of an existing member, you will use that information sheet to indicate whatever requires updating and submit that with your renewal dues. A new statement of faith will not be required if we still have your original on file. If we do not have this information, you will need to return to this page and proceed as if you were a new member.
