This form should be filled out by artists who would like to register their band name and group with moonkloud records. This will allow the individual artists on this form to participate in collaborative artist activities like band or group albums instead of just a solo artist album.

So for those of you who would like to participate in this type of activity just fill out this form below and submit it.

Approval of your request for a collaboration / band registration will be verified or denied within 5-10 Business days from receivel date of this order form.
