Play MYSA Soccer
2005 FALL SEASON (September through October):
Games will begin on or after September 12, 2005. Games will end at the end of October.
All games will be played at the Monticello Sports Complex (Hwy 278 West, toward Wilmar).
UNIFORMS:
Every player MUST wear shin guards to all practices and games. They are not provided by MYSA. Soccer Cleats are highly recommended but not required. Baseball cleats are acceptable with the front cleat removed. Shin guards must be completely covered by the socks. This year players will wear full game soccer uniforms. A full game uniform includes soccer jersey, shorts, and socks. If you need different size jersey and shorts, please indicate the short size on the application. Sock sizes will follow the ages below. If no size is provided, the following sizes will be applied:
4 & 5 year olds: Youth Small Shirt & Shorts, Youth Socks
6 & 7 year olds: Youth Medium Shirt & Shorts, Youth Socks
8 & 9 year olds: Youth Large Shirt & Shorts, Regular Socks
10 & 11 year olds: Adult Small Shirt & Shorts, Regular Socks
12 & 13 year olds: Adult Medium Shirt & Shorts, King Socks
14 & 15 year olds: Adult Large Shirt and Shorts, King Socks
FEES:
Make checks payable to �MYSA�. Registration fees include full game uniform. Returned check fee is $10. Applications received without payment, including checks returned by the bank, will not be considered.
AGES:
Girls and boys ages 4 to 14, based on their age as of August 31, 2005, may play. Please note that it has been difficult to form teams at the older age groups due to a limited number of applicants. MYSA will do whatever it can to accommodate the older players, but it is possible that we will have to cancel an age group. Please bring your friends!!!
REGISTER:
Return registration form and payment to address on the form.
VOLUNTEERS:
MYSA is a non-profit, volunteer organization. We can not maintain the league without parent involvement. We need coaches, Team Moms, and others. We also need referees (at least 13 years old with several years of soccer experience). Referees are paid based on the age group that they referee. If there are not an adequate number of coaches in an age group, the MYSA Board will try to find more coaches. If there are still not enough coaches, the Board reserves the right to draft teams without a coach, delay the draft until there are enough coaches, or reduce the number of teams in the age group. REQUESTS:
Most other sports do not allow requests. We work hard to balance the teams so that all teams have the opportunity to be successful. Special requests make our jobs more difficult. If you have a significant hardship, you may include your request on your form (please do not attach separate sheet). In addition, please ensure that both parties provide the same request (player A requests player B then player B needs to request player A). The MYSA Board and coaches will make the final decision for special requests when teams are drafted.
REFUNDS:
The MYSA board will review special circumstances. Generally, requests for refunds prior to the draft will be fully refunded. Requests after the draft and before the first game will be refunded less $25. No refunds after the first game.
