MEDICAL EVALUATIONS & MONITORING
(To improve the patients desire and show progress through health monitoring)
Establishing base lines, evaluating individual health needs, monitoring and tracking the progress and processes are a significant element in the success of any program.  We are prepared to use the CIMRO database program for Diabetes Quality Auditing.  Placing our identified patient base in workable groups where it benefits them to have a more intimate relationship with each other as well as the program leaders.

Each participant will have an initial evaluation with labs to establish base lines, any other testing that is necessary will be recommended at that time, including stress test for evaluation of physical condition, which will determine levels of exercise that can be tolerated.  These services are usually covered by insurance.  Services will be performed at Howard County Community Hospital and Medical Clinic.
Schedule of Follow Up
Labs taken and reviewed every three months
A1C
Lipid Panel
Data on exercise program gathered at each exercise session
Participation in group or individual sessions monitored as well as lecture series attendance
Using program residents in a rotation process, and supervised by Dr. Todd Paulsen PharmD; professor of pharmacology at UNL, all patients will have their medications evaluated, based on last lab detail.

A buliding on the campus of Howard County Community Hospital will be used for the Metabolic Syndrome Program.  The building is ideal; with a reception area and lobby that will be used as a group session area, and patient education area.  The building has six exam rooms that are sizeable; exercise equipment will be located in each room.  This will allow for privacy while patients are using the equipment, taking away the aspect of competition and often embarrassment that occurs in health club environments, thereby encouraging patients to exercise more.
