| With the participation of a LCSW, a Certified Diabetic Educator and nutritionist, as well as a Registered Nurse all patients will have an opportunity to discuss one on one or in a group environment the affects diabetes has on their lives. They will receive advice on choosing a mentoring partner to go through the program with them, learning what they learn and helping them adapt it to their personal life. Someone they can call on when the times get tough and the temptations to fall off the diet or exercise program are too great. Someone who can come and exercise with the patient, sit through the group or individual sessions, read and learn about an illness they may know nothing about. The CDE and RN are there for support with dietary education and diabetic education. Learning what to eat and when to eat are critical especially when you are adapting to a program of self strengthening and exercise. A resource library has been developed to provide patients the literature they need to educate themselves. The library contains videos, audiocassettes, brochures and pamphlets, as well as books to checkout. In addition to the group discussions patients can participate in the annual lecture series that Howard County Community Hospital and Medical Clinic sponsor. Speakers that are recommended by our providers are invited to speak on topics in their areas of expertise. This year we will try to lead the direction of the lecture series towards assisting the community in broadening their knowledge of the metabolic illnesses and their affects. |

| PHASE THREE EXERCISE AND HEALTHY ACTIVITY (Effecting life change by positive use of exercise and healthy activity) |

| Remaining active is essential for any weight control. Although the equipment will be setup for private exercise, we are hoping that the building will be utilized for group gatherings to socialize and share exercise time together. Pedometers will be distributed so patients can monitor their steps while fulfilling daily activities as well as participating in the program. Measurement of Body Mass Index will be part of the initial evaluation as well as the stress testing for evaluation of physical condition. Activity of any kind will be positive activity and we will promote golas based on individuals not standards set by others. Getting patients to exercise is essential and meeting their own goals is critical for the positive motivation that small successes bring. |
