Guide
* Couples intending to get married at Assumption Church are required
to fill an application form of Personal Data which is available at the Assumption
Church office.
* Couples intending to get married at Assumption Church are should:
- Notify the Parish Priest 6 months before the intended marriage
and book the date with the Parish Office.
- Make arrangements to attend the EE (Evenings for the Engaged)
programme.
- Make arrangements with the Parish Priest for the Pre-Nuptial
Inquiry.
- Register the marriage with the Civil Registration of Marriage
(AFTER filling up the Pre-Nuptial Inquiry form with the Parish Priest). An
Assistant Registrar of Marriage, Mr E.K. John is based at the St Francis Xavier
Church, 1435 Jalan Gasing. Mr E.K. John can be contacted at 603-7956 5977
during office hours.
* Copies of documents to be submitted to the parish office are:
- Copy of Baptism Certificate issued by the church (within the
last 6 months) where you were baptised.
- Copy of Confirmation Certificate issued by the church (within
the last 6 months) where you were confirmed (only if the confirmation has
not been endorsed into the Baptism Certificate).
- Copy of bride, groom and two witnesses' I/Cs.
- Copy of EE (Evenings for the Engaged) Certificate.
- Copy of Civil Registration of Marriage.
- Draft of the Wedding booklet - to be checked by the office before
printing.

PROCEDURE REGARDING CIVIL REGISTRATION OF MARRIAGE
- Fill up the Pre-Nuptial Inquiry form with your Parish Priest BEFORE
going for the Civil Registration.
- Fill up a registration form which includes a statutory declaration
before a Commissioner of Oaths/Marriage.
- Publication of banns of three consecutive weeks. The Civil Registration
office usually fixes a date 1 1/2 months after the three weeks grace period.
ITEMS YOU NEED TO BRING ALONG:
- Two passport-sized photographs each.
- RM20.00 stamp as payment for Marriage Certificate.
- Names of two witnesses and their I/C numbers.
- Other necessary documents.
You are advised to check with the Civil Registration office regarding
requirements, etc. as the information may change from time to time.
