We are looking for a tool to analyze the "state of affairs" in Knowledge
Management field at various companies and organizations. We would
appreciate very much any tip on where to find sample questionnaires that
can help to handle the matter. A questionnaire we are looking for should
not be directed to KM officers (as they do not exist in each
organization), but rather to an average member of staff/manager who does
not know what KM is. Best of all the questions should not explicitly refer
to Knowledge Management as such.

I am working on a similar project, trying to identify
knowledge management practices in portuguese
organisations, but with the difference that in my
study is just for small and medium business.
I am sending my questionnaire and hope it will help
you. I had the same idea about not mentioning the
expression "knowledge managemente" as it could be
confusing or mixed with Information Systems Management
concept.
The choice I made was to use a questionnaire used in
the "Pilot Knowledge management practices survey",
done back in 2001 by the Canadian statistics
Institution, which I found reliable. Then I changed
some questions in order to make it more adapted to the
portuguese reality, and added some other questions to
have some more detail, like the characterization of
the organisation.

As we are going to do similar studies, maybe we can
share the results or even compare it, as it could be

years ago which was directed towards KM officials, but may also contain
some questions that could be of interest to you.
