PRELIMINARY REQUIREMENTS FOR ADMISSION

1. The applicant must be Catholic.

2. The applicant must have a general average of eighty percent (80%) and above. Grade VI graduates applying for first year must have a general average of eighty-five percent (85%) and above.

3. The applicant must not have any failing grade in any subject and must not have a grade lower that S or its numerical equivalent in Conduct.

4. Initial screening shall be conducted by the Principal and the Registrar.

5. The applicant shall submit the following:

a. Original or photo copy of Birth Certificate

b. Baptismal Certificate

c. One recent I.D. picture

d. Form 138

e. Form 137 or Transcript of Record

Letter of recommendation from any of the following:

· Principal

· Teacher-Adviser

· Guidance Counselor

For Alien:

a. Alien Certificate of Registration

b. Updated ACR Receipt

c. Study permit from the Department of Education

Letter of Recommendation from any of the following:

· Principal

· Teacher-Adviser

· Guidance Counselor

A Grade VI student who leaves LSM will not be re-admitted. This provision does not apply to accelerants.


Admission Process


Applicants for High School must see the Registrar
Secure Application form and Guidance Questionnaire
Accomplished and submit Forms to the Registrar
Have interviews with the
Principal
Asst. Principal on Academics
Asst. Principal on Discipline
Secure testing schedule from the Registrar
Pay testing fee to the Cashier
On the testing day, proceed to the Office of the Psychometrician; show receipts and
application form.

After two weeks, inquire about the result from the Registrar's Office.
If qualified, secure application form for enrolment.
