Click on image to enlarge (Transcribed below) Application of Soldier, Sailor or Marine for a Pension Disabled by Reason of Disease or Other Infimities I, Ambrose Smith , a native of the State of Virginia , and now a citizen of Virginia, resident at Montebello in the County (or City) Nelson of in said State of Virginia, and who was a soldier in the State of Virginia, in the war between the United States and the Confederate States, do hereby apply for aid under an act of the General Assembly, approved March 7th, 1900, entitled "An act to give aid to soldiers, sailors, and marines disabled in the war between the States, and to every such soldier, sailor, or marine who by disease or other infirmities of age, is disable from earning or is without the means of procuring a support, and to the widows of Virginia soldiers, sailors or marines who lost their lives in said war in the military or naval service, or whose husbands have died since the war. "And I do solemnly swear that I was a member of 2nd Va. Cavalry Co. E and that I am now disabled now by reason of wounds & infirmitys of age and that by reason of such disability I am now entitled to receive, under said act, the sum of Fifteen dollars, annually. I further swear that I do not hold any National, State, or County office which pays me in salary or fees over three hundred dollars annum; nor have I an income from any other source which amounts to three hundred dollars; nor do I own in my own right nor does my wife own, property of the assessed value of more than one thousand dollars; nor do I receive aid or a pension from any other State or from the United States; and that I am not an inmate of a soldiers home.I do further swear that the following answers are true: 1st. What is the applicants age? Ans. 78 years 2d. What is the precise nature of the disability of the applicant? Ans. wounds & infimity incident to old age 3d. Is it total? Ans. no (a) Is it partial? And if so, to what extent does it disable him from manual labor? Ans. yes I can not do one fourth the work I once could Given under my hand this 2nd day of June , 190 0 I, W M Hite , a justice for the County of Nelson do certify that , Ambrose Smith whose name is signed in the foregoing application, personally appeared before me in my County (or City) aforesaid, and having the aforesaid application read to him and fully explained, as well as the statements and answers therein made, he, the said Ambrose Smith , made oath before me that the said statements and answers are true. In the County court of the County of Nelson on the27 th day of July , 190 1 The application of Ambrose Smith for a pension, with the certificate of the Chairman of the Confederate Pension Board of the County (or City) of Nelson , that it has been approved by said Board thereon endorsed, was presented to the court; and the clerk of this court is directed to certify a copy of this order to the auditor of public accounts. W.G. Loring, Judge County of Nelson I, E. L. Kiedel , Clerk of the Co. court of the county (or city) of Nelson , do certify that the foregoing is a true copy from the Records of said Court.
