II. DOCTOR :A PROFESSIONALIST

1. DOCTOR AS A PROFESSIONALIST

     The health of the patients should be doctors first consideration than that of a money. Recovery of health of his patients will give him return either in form of money, reputation, faith and extol.

     First the doctor must maintain recognition to his noble medical profession then he can hope for respect to his profession from others.

     Always respect the secrets which are confined in you. Never disclose one�s illness or weakness or disease or disability among others during gossiping or talks. The sick and weak person consulting the doctor has full confidence and faith in you, so never allow to loose patients confidence in you. If so happens, the patients may hate and hurt you [doctor] or may suit in court. Exception is always in the public interest or for welfare of society and human kind.

     Always practice the profession with conscience and dignity.

     Never mishandle any person physically, mentally or socially, at times that person may hurt you wherever he or she get opportunity.

     Do not blindly follow or copy or behave like other successful doctors, colleagues, teachers or seniors. Study their nature, behaviour and brain carefully and follow only good habits according to your personality and nature.The personality of each differs on certain reasons, inspiration, support or optimum situation in their lives and carrier. That you may not know or have.

     Professional progress is retarded by wrong decisions but right decisions require experience. So always be ready to accept ideas, advice and thoughts of others e.g. your colleagues, seniors and teachers, but be sure that they are not going to harm you in futhre.

     Services or professions done for glamour or under pressure of politicians, riches, antisocials (gundas) or for earning only money without reputation will not long lasting. A good and hard work, speak louder than your words.

     Boldness is the first, second and third things but it should aimed to welfare of your own patients. Never loose the temperaments in front of the patients, your colleagues and friends. A bed temper is a sign of inferiority complex. A hot temperament always proves costly whereas a cool disposition always pays.

     A hot temperament result from nature of the person. It can be won only by his own experience gradually. So always try to learn more from consequences of hot temperaments.

     Always remain with learned and progressive persons, get inspiration but never be jealous of them. Always appreciate and encourage the sincere approach, effort and performance.

2. NORMS TO DEAL WITH PATIENTS IN GENERAL

     Always take a complete history and do thorough examination of every patients. Never consider these of casual importance but establish and adhere to a routine as you have been trained for.

     Patients must feel that your full attention and serious consideration are being concentrated upon him, and his problem are important to you. Neverlet him feel that you have examined him in haste. Avoid un-necessary interruptions.

     Never make fun with a patient concerning an �ailment� and listen his complaints carefully. You should realize that patient comes to you only when he thinks his trouble or complaints seriously. If the patient has no organic disease after your thorough examination and investigation, do not hesitate to reassure him and convince him completely.

     Make positive statements on history and about normal findings to the patient. Use positive approach and do not describe the results as negative.

     In case of minimal or irreversible disease, a sincere re-assurance based on careful thorough examination is best medicine for many patients.

     Always and do for patients exactly same what you would choose for your self or your family.

     Do not charge excessively for your services, on the contrary also remember, free consultation to be rarely followed. The patients who receive free medicine or surgical treatment, often become dissatisfied and also imagines to be neglected and incomplete care. So charge reasonably and minimum in considerable patients of obligation.

     Never demand forextra fee if other family members consult you just for a vague symptoms or simple disease, after you have examined the main patient for which you have been called for home visit. If you do not wish to examine the extra patient at the same time, simply ask them with smile to attend your clinic or at residence for proper and thorough examination. Never allow them to feel your impression to be of uncooperative.

     Never demand the fee of your consultation or home visit before you reach for visit or during your examination of a serious patient. Remember nonacceptance of fee in such situation will enhance your reputation and image in long run and pays you not financial but in other forms.

     If you fail in diagnosing the disease or do not able to treat the patient, then think more than once and try again and again. If still do not succeed, never hesitate to consult your colleagues, seniors and even juniors at a times for the benefit of patients. If needed, the patient may be referred to other higher centre or to an experienced doctor. Never make it a prestige issue. If you think you can not succeed, never feel ashamed to accept your failure.

     Your work and behaviour with patients or their relative always must be reasonably excellent. Do not take undue advantage of patient's faith in you.

     Allowing the facilities too much to the patients which are entirely unjustified, will not necessarily help you in increasing your reputation.

     Your talent of skill should always be utilized for the benefit of your patient and human kind. Do not take undue advantages of your better quality or sevice. You should always be reasonable, compare with your colleagues or competitors, it will keep you always ahead of them.

3. DEALING WITH POOR, RICH AND MEN IN INFLUENCE

     Free medicines or drugs [physician sample] are for poor folk. Providing free medicine to politicians, rich, persons in influence is troublesome to doctors many times. These persons take these facilities as their rights later on. In case of full or partial non-availability or interruption in these facilities will annoy these persons and they will become enemy in future.

     Poor should be attended with sympathy, soft voice and consolence. These persons are source of your advertisement and extol in the community. These are your indirect force to counteract any ill-happening. Be always ready to serve the poor by every means.

     On the contrary never neglect the rich, influential persons, officers and politicians otherwise these will develop possibilities of hampering your progress.

     Always be cooperative and tolerant with rich, influential or renowned persons. Listen them calmly and evaluate their comment carefully.

     Always try to minimize your differences with those in-authority or person-in-influence. Avoid any unreasonable and unnecessary approach to them. Your patience and polite dealing will save your abnormal behaviour, otherwise they may push you in awkward position and in some more trouble.

     Do not run after and waste your time to develop intimate relationship with egregious or person in power or influential politicians unprovokedly. No harm on contact if really needed.

     If your all round performance or behaviour are good then you will find them accessible when you really need them. Moreover while dealing with these persons, respect them for which they deserve. Appreciate and try to cooperate them. Be tolerant in conforming to their requirements. It will help to speed up your work and maintain your progress.

     Deal labour class always by convincing that the treatment prescribed to him is appropriate and is best of his knowledge and experience. Rough dealing to labour class may degnigrate you in the society.

     Do not unnecessarily confront with antisocials. Their roots may be strong and may be related with some politicians or other man in power. You being a professionalist should not waste the time either in encouraging them nor in their opposition.

4. DEALING WITH AGED PERSONS, CHILDREN AND HANDICAPPED PERSONS

     Aged persons should be dealt first with due regards, utmost care and sincerely. Persons with 60 years or old are fast growing segment of our population, constituting about 6-12 % of total population.

     Older people suffer from many of the same diseases seen in younger people, but the elderly have different presentation and chronic coexistence of several disorders. Thus require a combinations of medical care and approaches quite different from those of younger.

     The aged person should be listened carefully. They may be time consuming because of slow and disarticulation. Moreover, some of them may be psychologically disturbed because of their neglectation in family. Remember, aged persons are cooperative to carry out treatment and instruction regularly and efficiently.

     Aged person needs respect from their own children rather than wealth. Like wise they need to be consolated by doctors rather than treatment.

     Always try to avoid inflicting pain by gentle manipulation. Explain the person the process very well which you are going to do. Use local anaesthesia whenever necessary. If a aged person cry and abuses due to momentary pain during essential part of examination, take yourself easy and never loose the temperament.

     During examination of the child, always take the parents in confidence and try to be in, easy and jocky mood, although remember that you must be serious in diagnosing the diseased part or ailment.

     If a mild or momentary pain is un-avoidable, warn the child, take his consent with affection [in case of elder child, and too small child, consent of parents] and then proceeds for examination. If you hurt him [specially child] after promising or assuring not to do so, he will never forgive or forget.

     A handicapped person either physically or mentally should be attended first, with sympathy and personal attention. Here pay your kind attention more than others.

     A blind or deaf person may take your more time in history taking and examination. Listen them carefully. Keep patience and satisfy them with reasonable assurance about their illness.

5. DEALING WITH RELATIVES AND KNOWN PERSONS

     Never prescribe the medicine on only by taking the causal history of the relative or any known sick persons just because he is not paying. Always examine him thoroughly and pay proper attention and then prescribe treatment. It will definitely give return in some form if not in cash.

     Never allow to develop a bad opinion about you for your improper attention, carelessness dis-courteous attitude towards relatives and known persons.

     Never show your anger even if any relative try to consult you about some illness in general social function or gatherings as it happens mostly. Listen them peacefully and convince them with smile to attend the clinic, hospital or residence as complete examination and proper investigation are essential before prescribing the medicine. Don�t allow to run the clinic in social gatherings.

     Never hesitate to take your reasonable due fee or money which you have spend on your relative if it seems to be desirable and appropriate. Never show your unnecessary obligation on them. Reasonable fee will keep your relatives tension free. They will never hesitate in consulting you in future, they will always follow your instructions faithfully during treatment.

     In case you have not charged your due fee, or they have not paid, never discuss or exhibit this liberalness among other relatives and in the society to prevent you critics.

     Remember, your attendance in serious illness of your relative is boon (good blessings or asset to the family). You should justify your duty even without a call from them in such odd situation.
