Next, Scroll or Read to the bottom of the page and click on the lightning bolt button there. You will be taken to a pre-evidence information page. 4. If you would like to view an example of the federal complaint form to be used , click on the blue "CLICK HERE" link there. 5. Select the Application that is most suitable to your needs based on whether you are applying as an individual (includes husband and wife or significant other) or as a representative for a legal entity such as a corporation. 4. Print out the Application desired. Also print out the Web Site Consent Form that is there. 6. Complete the Application and contracts required to be printed, and then Fax the Application and contracts as directed there. 7. Payment procedures for joining will also be provided there. 8.
