Amy's Little Angels Daycare Forms

On this page you will find references to and samples of all the different forms I needed to start my Home Daycare Business. Feel free to download them. All are in .PDF (Adobe Acrobat), so if you need the reader, click the link at the bottom of the page to download it.

Some of the forms here are specific to the Commonwealth of Pennsylvania. However, I am sure that your state has similar forms. I would recommend browsing your state's website.

My Daycare Forms

  Fictitious Name Registration Form:  This form is used to register your Daycare Business Name with the State. It is required if you will be operating anything but a Sole-Proprietorship type of business. This form is for PA and is in Adobe Acrobat.

  Daily Check-In and Check-Out Form:  This is required by most states if you are going to receive subsidy payments. However, it's just plain good sense to have a record of when the child was brought into and out of daycare.
  Permission to Photograph Form:  This form is a good way to make sure that the parent(s) do not mind you taking pictures of their children and using it for posters, crafts, or on your website (not recommended). However, taking pictures is good for activities like Mother's Day cards or Christmas cards.
 Telephone Interview Form:  An excellent way to take quick notes during that first contact with a prospective customer/parent.
 Emergency Contact Form:  Gotta have these! Whats, whens, wheres, whys in case something terrible happens. Quick reference for who to call and where to go.
 Infant Daily Summary Form:  If you will be providing for infants, the parent(s) will need to know how much they ate, when they slept, and many other bits of information.
 Notice of Withdrawal:  This form is handy when you (or the parent(s) need to terminate the providers services.
 Change in Schedule Form:  If the hours or service change (by the parent(s) or the provider), this form is a good way of getting the information to the parent(s).
 Permission to Pick Up Form:  This form is used to allow the parent(s) to designate alternative persons who can pick up their child(ren) if they are not able to. It really helps to protect provider, parent, and especially the child(ren).
 Field Trip Permission Form:  This form allows the parent to specify if their child(ren) may accompany the other children on outings, or if the provider must call first, or not go at all.
