The bottom lever of the heater box was broken.  This apparently is fairly common for this year.
A kit is available from aftermarket companies to repair this lever.
The first step to install the kit is to grind the rivets flush with the retaining plate.
Then, the front rivet must be ground off of the control lever.
Once this is done, the new lever is ready to be installed.
The front of the lever is attached with a washer and self tapping screw.
The back of the lever slides onto the pivot rod and then the retaining plate can be re-installed.
To secure the retaining plate, center punch the rivet posts.
Then drill two holes to accept the provided self tapping screws.
Once the screws and washers are installed, your control unit is ready to clean up and go back into your car.
