Finally, the whole assembly is back together again and everything looks good.  All the leafs and in a progressive order on both sides.  I placed some grease on both sides of the new leaf to help prevent squeaking.  I have learn now that the grease did not do much.  The entire truck sqeaks now pretty good.  I have been advised to place some anti-seeze lubricant instead.  I will update this page when I find out if it worked or not.
I made a mistake and cut off one of the two leaf holders.  The directions showed me using the new clamps to keep the stack together.  After installing one new clamp, I noticed I could bend back the factory clamps and there was enough room to hold the new leaf in the stack.  I also felt the factory clamps with the rubber on the sides did a better job holding the stack together rather than the new ones Rancho had supplied.  Later I found out I needed that new bracket for the front leafs and had to remove it anyways.  Either I was short some clamps for the rear or I just mixed them up.  Either way, I only had enough clamps to hold the front or the back and they were more necessary for the front.  I eventually put the old one back on but without it being attached to the leaf like the other three.  I simply used a couple of C-clamps to wrap the holder back around the leaf stack.

Here is a shot of the truck with the new 1-1/2" add-a-leaf and the 1/2" block in place.
