The following must be included before registering: -Birth Certificate -Immunization Records -Social Security Card (Some form of I.D. if not U.S. citizen) -Proof of Domicile (Rental Agreement, phone or utilities bill, mortgage agreement) -Transcript of Grades -All studends must be accompanied by a parent or gaurdian (With papers) If you are unable to supply the above information we will be happy to reschedule your appointment. |
