Drs. Prolla and Diehl's INTERESTING CASE OF THE MONTH October 2003 case A
FNA inguinal node: lymphocytic lymphoma

500x Papanicolaou staining, poor fixation, small monotonous lymphocytes, with some chromatin clumping

500x M-G-G, little variation in size and shape of lymphocytes

500x M-G-G staining

500x M-G-G staining, most of the cells are small mature looking lymphocytes

The smears show a monotonous appearance, with abundant number of lymphocytes, normal looking, with very little variability in their nuclei, and absent nucleoli. A few scattered larger lymphoid cells were seen (prolymphocytes). No mitotic figures were seen. No dendritic cells with tingible bodies were seen. The lymph node biopsy showed well differentiated small cell lymphocytic lymphoma, with B cell type of immunocytochemistry reactivity ( CD20 +, and CD23 -). Because of this CD23 negativity, some people would call this case an "atypical SLL". Some of my readers made the diagnosis of spermatocytic seminoma, a good diagnostic alternative. One problem with this case was the not so well stained Papanicolaou smears. Day by day life is full of imperfections...
