| Questions Parents should be prepared to answer upon initial visit to speech-language pathologist. 1. Mother's pregnancy history 2. Child's birth history 3. Hospitalization's, surgery, etc.: 4. Family history of late talkers, speech-language deficits: 5. History of ear infections, tube? 6. Does the child attend a daycare or program or stay at home? 7. Other siblings in the home? 8. Other development milestones (crawling, walking, cooing, babbling)? 9. How does child currently get their wants and needs met( gestures, grunting, crying, anticipated by parent)? 10. Does child display any other behaviors (crying, hitting, acting out)? 11. Has your child been diagnosed with any disorder (i.e. autism, pervasive development disorders, Cerebral Palsy, etc.)? 12. Any additional concerns the parents have? 13. Medications? If you have any questions for Ms. Glenda, she has suggested that you send them to my e-mail address.
