Please complete all sections.  do not be modest. Every Bit of information can be used by the Faculty Council to assist with the selection process.  Completion of this form does not guarantee selection.

I. Current Activities: List all activities in which you have participated during high school.  Include clubs, teams, musical groups etc., list accomplishments in each.

II. Leadership Positions: list all elected or appointed positions held in school, clubs, community or any other organization. only those positions in which you are directly responsible for directing or motivation others should be included. ie; team captain, club officer.

III. Community Activities: list community activities in which you have participated and note any major accomplishments.  These should be activities outside of school in which you participated for the betterment of the community. ie: church groups, Scouts, etc.

IV. Other:  list any job experiences, honors, or recognitions that you have received which support your bid to be selected for membership in the National Honor Society
