| Conditions & Terms: Final amount is due by the end of the event. Please make all checks payable to "Hoops DJ Service". The contact person is responsible for assuring that the event location has at least two (2) 110 outlets accessible to the DJ and at least two (2) six (6) foot tables for the DJ to set-up on. Hoops DJ Service shall not be responsible for any injuries sustained to any guests and/or event workers during the event. Hoops DJ Service representatives hold the right to discontinue and/or not begin a performance if they feel that the performance area provided is not a reasonably safe environment, or if the environment becomes unsafe during the event. (This includes facility and/or guest hazards). If Hoops DJ Service representatives feel that the facilities and/or guests have become unsafe for performance, the contact person shall be notified immediately, if they are available. Cancellations:Any cancellation up to 31 days before event (Except December Events): Hoops retains deposit of $25.00Cancel 30 days or less before event (Except December Events): Contact pays 50% balance��unless new booking occurs) December Events: Cancellation received 60 days or less before event:Contact pays FULL BALANCE In cases of tragedy, (loss of loved one) exceptions can be made, on an individual basis |
