| How To Get a Yellow Card |
| When you sign up for sports in the Nurse's Office, we ask you to indicate whether you expect to have a private or school physical. Your health records are reviewed and you are asked to stop by the Nurse's Office to be informed as to what forms you may need to get a yellow card. Sports Physicals are good for one year 1. Physical exams for Winter start October 1st 2. Physical exams for Spring start February 1st 3. Physical exams for Fall start May 1st Blue Sports Sign Up Sheet listing four catagories of participation: Collision Sports Contact Sports Endurance Activities Other Football Baseball Swimming Golf Ice Hockey Basketball Track and Field Cheerleading Soccer Tennis Wrestling Volleyball Softball Cross County Skiing Diving The examining Physician should Initial the box above each group of sports in which participation is allowed. School Physical exam form to be used for private physicals Parent Permission Slip is needed for each sport during the year You may be asked to provide clearance from an M.D. due to recent injury, orthopedic problem or long term illness.* A Tetanus Booster will be needed if you have not had one within ten (10) years. A vision report and diopter readings must be submitted if student wears glasses or contact lenses in order to participate in a contact or collision sport.* If a student signs up for a school physical, he/she has to come to the Nurse's Office for blood pressure and urine screenings. If a problem is indicated by either of these tests, or if a problem is identified by the school doctor as a result of the physical, the student is referred to his/her private physician or clinic before a yellow card can be issued. All private physicals must be reviewed and co-signed by the school doctor assigned to that particular school. |
