prescriptions?
Many individuals
are choosing daily
between buying food or prescription medicines.
Western Maryland Prescription Program provides access to prescription
medicines
to
qualified individuals.
Who is
eligible?
Residents of
Allegany,
Garrett and
for their medical conditions.
Individuals who have no prescription coverage
by private
insurance or Medicaid and
meet the financial guidelines.
How to
apply....
Individuals,
physician
offices, free clinics, health departments, Department of Social
Services and other community organizations may make referrals.
Referrals
are
accepted by phone, fax, mail or in person.
How the
program
works....
1) A program
representative will
complete an application for each referral to
determine eligibility. This includes a financial questionaire.
2) Completed
forms
will be mailed to the physician for signatures when required.
3) The
application is
then forwarded to the Pharmaceutical Company with the
physician's prescription.
4)
Prescriptions are
sent directly to the physician for dispensing and education.
5) A program
representative will follow up to assure receipt of medicine and refill
needs.
How long
does this
take?
6 to 8 weeks
from
application to delivery.
What
information
do I need to complete an application?

Social Security
number or green card
·
Verification
of income
·
Statement
of benefits letter from Social Security stating benefit amount
·
Copy
of prescription applicant is unable to buy
·
Copy
of denial letter if Medicaid application was filed
·
List
of assets ( home, savings, checking, CD's, IRA's, etc. )
·
Pharmacy
printout of medications including the cost
