This is not an official government form, but it is an efficient tool which can be used to aid prompt action. The form can be submitted without all fields completed. Most people's stories are narratives, but when you browse the form it will remind you about important details. Tip: Get the identifying information first - names, ID numbers, license numbers, employment positions - other info can usually be filled in later, but don't delay.

Tell the truth.

6. Briefly state what the offender(s) did. If the offender issued a citation, include the statements contained in it. If you were confronted by the police, be sure to indicate if the police told you specifically who called them, and who the callers were.
