VISITING FOR BUSINESS OR VACATION?

Requesting a temporary transfer to Honolulu or Hilo:

1. Have your clinic fill out this Transfer Form (click to download). "Home clinic" is your clinic. Kû Aloha will fill in Physician's signature, Client number, Kû Aloha counselor, Date and Fee.

2. Fax form to Kû Aloha attention: COUNSELOR - 1130 N. Nimitz Hwy. Ste. C-302, Honolulu, HI 96818 for Honolulu; and Brian - 900 Lelani Street, Hilo, HI for the Big Island.

3. Have your clinic follow up with a phone call to Kû Aloha to verify the information.

4. Kû Aloha will notify your clinic of approval and/or special arrangements, if needed. Your clinic will also be notified of the fee.

Transfer procedures:

1. Send check or money order to Kû Aloha to arrive prior to your first clinic visit at Kû Aloha; OR bring cash or money order at your first clinic visit.
   The fees for methadone are: $30 the first day and $25 each day thereafter.

   The addresses for the respective clinics are listed above. Please send your check to the clinic where you will be dosing.

2. You will need to bring with you:
   1. an official photo identification card
   2. a copy of this letter
   3. your home clinic bottles (if you have take-homes)
   4. cash, or money order unless a check was sent previously

Directions:

Honolulu site
Kû Aloha is located 10 minutes from the airport by car. If driving, take the Honolulu exit out of the airport going eastbound toward Diamond Head. Once on the freeway, take the Waikiki exit on to Nimitz Highway. Go straight on Nimitz until you see a green building complex on your left with large yellow metallic structures. Turn left at the complex on to Ala Kawa. Kû Aloha is in the building at back of the building complex on the mountainside. Take the stairs or elevator to the third floor and Kû Aloha is right there.

By cab, Kû Aloha is approximately $18.00 from the airport.

Hilo site
Kû Aloha is located 5 minutes from the airport by car. If driving, leave the airport and turn left at the first traffic light you come upon. Stay in the left lane. Take the first left on to Leilani Street. Go straight ahead until the road splits into two. Kû Aloha is the two story blue building located right at the juncture. Turn into the parking lot on Leilani Street.

1. Sunscreen is a must, especially if you're planning to go to the beach.
2. Call Kû Aloha when you arrive so we know you got here safely! And we can give you directions from your hotel, if need be.
