1) It is not clear whether those are the same ship. It is possible the wreckage of the (obviously newly comissioned) ship was recovered after Wolf 359. Encyclopedia 2.5 shipchart takes the 71620, and since this registry was clearly visible on an Okudagram, but the model was not, we can assume the registry is NCC-71620.
2) The Fact Files are the only source for the ship. I assume the original registry was NX-62497. The original sketches show the vessel USS Akira with the registry 2497.
3) The Encyclopedia is the only source. I assume the original registry was NX-10521.
4) If this is the class ship, the former registry may have been NX-501. But other than Naren Shankar, I don't assume the bajoran freighters and this vessel had anything in common. :-)
5) The Encyclopedia lists this as Olympic-class, but it is clearly visible that the ship is an Oberth-class vessel.
6) The ship was not mentioned in 'Endgame', but the same CGI-model, still labeled 'USS Bonchune', was reused for the final shot of the show.
7) The ship bore both registry numbers, but since this was ment to be a hommage to the movie '1941', it is safe to assume the NCC-1841 was an error. It is not known if the ship was the same vessel as the one mentioned in 'Cause and Effect', but until stated otherwise, I simply assume it is.
8) The model was mislabeled 'USS Brittain'.
9) The Encyclopedia says it was a Daedalus-class ship, but since the class went out of service in 2196, this should be impossible.
10) The Deep Space Nine Technical Manual says it is an Excelsior-class variant, but until stated otherwise, I assume this is a yet unknown class.
11) One could assume that NCC-2032 was the class-ship, but since the model at Wolf 359 was a Galaxy-class-kitbash, this seems impossible.
12) Interestingly, the registry is lower than USS Yamato's which was said to be the second Galaxy-class ship. If the starship Challenger followed the chronological production order, Enterprise-D would only be the 4th Galaxy-class ship (counting USS Ron Moore, it would be the 5th.)
13) The starship Columbia from the com-chatter in TMP was a Cygnus-class ship according to Franz Joseph's Star Fleet Technical Manual.
14) The Encyclopedia gives both names, 'USS Concord', (named after the city) and 'USS Concorde', (named after the plane).
15) The wallchart in TUC lists the original registry, NX-1974. It is not entirely clear wheter the ship from DS9 is the old Constellation-class Prototype.
16) The NCC-1700 comes from a display in 'Space Seed', but I haven't found the display or the name/number yet. The number later appears on the wallchart in Commodore Stone's office in 'Court Material'. The Franz Joseph-picture of the USS Constitution NCC-1700 appeared as the lifesign-display in ST:TSFS and later as part of the 'fileoverload' in several  early TNG-episodes.
17) The DS9TM says it is an Excelsior / Constitution-class variant, and the name 'Shelly-class' has been established as the inofficial name in the fandom, but still, until we get an official name for the ship, I assume this is a yet unknown class.
18) I simply can't ignore the fact that there was a starship NX-01-A in 'Hope and Fear', VOY, and no one of the crew seemed to be surprised or even irritated since the original NX-01 is a starship called 'Enterprise'. The only valuable explanation could be the fact that the old earth starfleet had been somehow 'assimilated' into the new Federation Starfleet after its foundation in 2161. Thus, the old earth starfleet registries may have been rearranged. As long as this isn't explained or solved in any way, I assume there was a predecessor for the faked NX-01-A, and it was the NX-01, USS Dauntless.
19) The two Defiant-class ships in 'Message in a Bottle' had no names, but since the CGI-ships were still labeled 'USS Defiant NX-74205', I assume at least one of those ships was the USS Defiant.
20) The ship was still labeled NX-74205, allthough it should have been something like 'NCC-74205-A'. The cause was the reuse of old CGI-sequences and models for the final battle. The ships in 'Endgame' probably were still labeled 'USS Defiant NX-74205', too, but since the first Defiant had been destroyed by the Breen, I assume one of those ships was the second Defiant.
21) The class of the ship, Constitution, is from the Star Trek Encyclopedia.
22) The class of the ship, Constitution, is from the Star Trek Encyclopedia.
23) There were plans for ST:FC to create a new USS Endeavour, the Admiral's flagship. However, those plans were never developed and the new Endeavour only existed as a sketch. This is the reason I assume the Endeavour fighting the Borg in 2373 was still the Nebula-class ship.
24) The Entente is one of the Federation-class Dreadnoughts from Franz Josephs book. The class was never mentioed on-screen, so (other than the Hermes / Saladin / Ptolemy, see other entry) I cannot concider it caonon at the moment (allthough it is my believe that the ship is a Federation-class vessel.).
25) Allthough the ship was already destroyed in TVH, we saw the destruction sequence again in the senate chamber. The ship was often mentioned in TNG, DS9 and VOY, and the Enterprise-D and Enterprise-E both had the model ship in the observation lounge, so it seems nearly impossible to count all the appearances after TOS and the movies. I just leave this open.
26) The ship was a variant with several modifications.
27) The Enterprise-E wasn't seen, but mentioned in dialogue on Voyager.
28) The Excelsior from 'Interface' could have been a newer ship of the same name, but the Encyclopedia suggests it to be the old ship. The CGI-model created for 'Flashback' was labeled 'USS Excelsior', of course, but I doubt the name was changed for the ship's next appearance in the large fleet in 'Favor the Bold' / 'Sacrifice of Angels' (and probably other episodes of the war-arc). And since the original Excelsior still existed in 'Interface' it could very well be the ship also fought in the fleet to liberate DS9. The Excelsior's registry in ST:TSFS was NX-2000 and maybe in ST:TVH, too.
29) The Excelsior-class has never been made 'official', but the Encyclopedia speculates it was the Excelsior seen at the end of the episode, the predecessor of the Nebula-class ship reactivated for duty after the newer ship had been destroyed by the Klingons. There are, however, sources that suggest the Farragut mentioned in the episode couls also have been the 'Yeager-class' ship seen in the background of the same shot. Until anything further is known, I'll go with the Encyclopedia.
30)  The Fredrickson the damaged Excelsior in 'A Time to Stand'. Whether the ship appeared in 'Sacrifice of Angels' is not clear. It was the ship docked at Utopia Planitia in 'Relativity', allthough then obviously CGI.
31) The Encyclopedia still gives the NX-70637, but the appendix already lists the NCC. The ship was one of those Galaxy's seen at Chin'toka, at that point the ship already had the NCC-registry.
32) It is not clear if the USS Gettysburg from DS9 is the same ship as the one mentioned in TNG.
33) Again, I don't think that USS Hermes (NCC-10376) looks like the original USS Antares featured in 'Charlie X'. The ship would then have the same lifespan as the Oberth, probably even more. Either those are two different Starfleet ship classes (one Antares-class more or less doesn't make a difference ;-) ) or the ship has been heavily upgraded.
34) As far as I know, the ship only apperaed in the starship chart appendix of the Encyclopedia 2.0. After the re-release with 128 additional pages, the ship had been replaced with the USS Appalachia. The reasons are still a mystery.
35) The ship in 'Tears of the Prophets' was the USS Hood, allthough the ship had the USS Lakota's registry. We may assume the Hood's saucer was heavily damaged and had to be replaced with the USS Lakota's sometime prior to 'Tears of the Prophets'. :-)
36) It is not clear if the ship Geordi mentions in 'Force of Nature' is the Intrepid-Prototype or still the old Excelsior. The episode too place at Stardate 47310, USS Voyager (the second Intrepid-class ship) was launched several month later, Stardate  48038. Assuming that the USS Intrepid already existed at that time, it seems plausible the ship from 'Force of Nature' was the new Intrepid. The Prototype-Intrepid's registry should be NCC-74600, according to Rick Sternbach.
37)No idea where this name originates (obviously Okuda, from what I found out), but this seems to be one of the ships seen in the war-arc.
38) This could be the prototype of the Korolev-class.
39) The ship was the Excelsior-variant first seen as the USS Enterprise-B.
40) The Leeds was the ship from the new 4th seson-intro. The ship may have been present in later battles such as the Borg-battle in 2373 since the model was probably not relabeled for the movie. Take this as inofficial until the question if the ship was relabeled is solved.
41) The Encyclopedia could be wrong here. Personally, I doubt there was an Excelsior-class Lexington (or, at least, it has never been mentioned) because the Nebula-class ship was already in servic 3 years prior to the episode. And the registry even seems to predate this. Even if the Excelsior was decommisioned the same day she met the Enterprise-D, the timeslot wouldn't be large enough. And we saw the Nebula docked at DS9 but not the Excelsior. Take this however you want, but until the next version of the Encyclopedia corrects the error, the Excelsior-class ship will stay in the chart. The Lexington being in FC is a part of the com-chatter. A voice said '42 wounded on the Lexington'.
42) 'The Art of Star Trek' pictures the wrecked shuttle 'Kotoi' from the starship Liberator, a model obviously present at the graveyard.
43) Maybe the Magellan from 'Sacrifice of Angels' is another ship. Sisko mentioned it in connection with the Venture, another Galaxy, and the 'Galaxy-wings'. Until stated otherwise, the ship stays the old Constellation, allthough I doubt this, too.
44) From the com-chatter. It is not entirely clear wheter the voice said 'Madison' or 'Manson', but it sounds more like 'Manson'.
45) It really doesn't make sense to have two ships, same name, same registry, fighting in the same battle, but different classes. I could take the Excelsior as an 'accident', or the Nebula, but involving my own theory is as good as completely ignoring the ships and all inconsistences. Since they asked Riker to take over the command of the ship (they didn't say which Melbourne, so they could have ment the newer Nebula class, probably just ready for entering service), we might assume the old Excelsior was going to be decommissioned. Maybe the Nebula (which had modified arrangements insread of a 'pod') was a prototype vessel, some sort of enhanced Anti-Borg-Cruiser. To cover it's existence both ships, the Excelsior and the Nebula, bore the same registry. During Rikers conversation with Admiral Hanson, the display screen showed another (registry?) number, 48273. This could have been the Excelsior's original number, if we assume Hanson commanded the Excelsior-class Melbourne during the battle (the same ship that carried him to his meeting with Picard at the beginning of the episode). However, this is highly non-canon, so agian it's up to you to interprete the situation. But this is my version. Additionally, if the Nebula was a new ship, we probably 'saw' the Excelsior in '11001001'. Maybe there was some Okudagram in the control center of the starbase in '11001001' where the registry was listed, too, but we can't be sure.
46) The Nebula-Melbourne's model was present during 'Future Imperfect' in Captain Riker's ready room. This further supports the theory that Starfleet wanted him to assume the command of the Nebula-class ship, not the Excelsior. Obviously, the wrecked ship was also present during 'Emissary'. The wreck was later seen in the junkyard of Qualor II. This could have been another ship of similar design, but going with the idea that the Nebula-Melbourne was a prototype (see above), this was presumably the starship Melbourne, too.
47) Obviously, Franz Joseph's technical manual lists a ship of this name and number as a member of the 'Bonhomme Richard-class'. I can't confirm this, so I laeve it open. See 'USS Entente'-entry. The Merrimack is only in the Director's Edition of ST:TMP.
48) I can't confirm 'Birthright', the Encaclopedia doesn't list it, but several other shiplists do. If the ship was only mentioned on the 'operations chart', a display showing several ship operations in the sector, I had to add another 20 episodes to the episode-chart, for this ship and a dozen others. 8-{
49) This seems to be a whole group of vessels, several modifications and variants. Some call the 'Mother-Miranda', the USS Reliant, an Avenger-class starship, but as always the Encyclopedia is our bible and it says it is a Miranda-class ship. So Miranda is official.  :-/
50) Argh...!
51) The TNGTM pictures several concepts for the planned Galaxy-successor, the Nova-class. Judging by it's registry, the Nova from Voyager is an elder design, so maybe the developement for the class began a decade ago. TNGTM makes no clear statement, so we can only assume the Nova-classes are the same.
52) The wreckage on the planet's surface was a reuse of parts of the demolishoned USS Enterprise from ST:TSFS. In other words: the Olympia probably was a Constitution-class ship.
53) The Pasteur's model was labeled 'NCC-58925' while the dedication plaque showed 'NCC-58928'. The Encyclopedia took the 58925 as 'official' and so do I.
54) An Okudagram in engineering showed a four-nacelled vessel. The Okudagram was a survivor of pre-production, then they intended to use the Cheyenne-class model instead of the old Oberth.
55) Obviously, the TUC-chart showed the errendous registry 'NCC-1711'. All other sources give the 'NCC-1657' as registry, so I'll take it as official.
56) The wrecked ship at Qualor II could als have been another Princeton-class vessel.
57) Both the ship's MSD and the dedication plaque show the 'NX-74913' while the hull is labeled 'NX-59650'. Allthough I assume the 59650 is just a countermeasure to reveal that the Prometheus is actually a new prototype vessel, the Encyclopedia and all other sources take the NX-59650 as canon. So I added both numbers to the registry chart. Whether the NX-59650 is the right registry or not is uncertain, but only the fact that there was a Nebula-class Prometheus with a registry higher than 70,000 leads to the assumption the lower one is the false registry, an error.
58) The Encyclopedia gives the registry NCC-59983, but another registry is clearly visible on a screen during the episode.
59) The vessel Raven is labeled SS Raven NAR-32450 in 'The Raven', meaning this would have been a civilian ship. Later, the CGI (the ship in 'The Raven' was a matte-painting) was renamed USS Raven and had the proper NCC-listing. Since we saw the USS Raven clearly while the SS Raven was just a wrecked debries-field, I assume the ship was indeed the USS Raven.
60) The dedication plaque gave the registry NCV-474439-G and said it was a Wells-class timeship, built at the 'University of Copernicus'. I listed it here because the plaque said it was a UFP-ship. There was, however, a small error because the ship was obviously not 'the seventh ship to bear the name', but the 8th. The dedication was "The only reason for time is so that everything doesn't happen all at once." a quote by E.M. Rauch.
61) Allthough not official, the author of the episode, Ron Moore, said this was ment to be the old ship from TOS.
62) The Revere was a ship from Franz Joseph's technical manual, a Hermes-class vessel. This has never been made official. Nontheless, the existence of the Hermes-, Saladin- and Ptolemy-classes can't be excluded from the official Star Trek-universe. (See notes )
63) Obviously, this was a variant.
64) The USS Ron Moore is from the DS9 '01-calendar. It was the ship docked at DS9. Whether this ship appeared on DS9 or not is uncertain, but if it did, we'll probably find it at Chin'toka in 'Tears of the Prophets' since it appeared to be CG. There was, however, another scene showing a Galaxy-class vessel in 'Image in the Sand' and 'Shadows and Symbols' docked at DS9 which appeared to be model-work instead of CGI.
65) A variant of the Miranda-class. A model of the ship was present in Sisko's office, as was a model of the USS Horizon and a Nebula-class variant with a still unknown name.
66) Allthough the FactFiles confirmed the name 'USS Spector' for the ship that appeared in Voyager's 'Message in a Bottle', the actual CGI was probably still named 'USS Thunderchild'.
67) The ship in Picard's Ready Room was not the USS Stargazer; it bore the registry NCC-7100 but no name.
68) Allthough the Tian An Men was reported lost while patroling the cardassian border, the ship later appeared in 'Tears of the Prophets'. So either the ship from 'Tears of the Prophets' was the replacement of the same class (we never got a close-up of the registry ;-)) or the ship was later, after 'In the Cards', recovered.
69) Since the official site says there was a starship 'Trial' in the fleet at the end of the episode, it was either one of the two Excelsiors or the Miranda-class vesseldocking at the station.
70) The model of the starship Trinculo is present at a Planet Hollywood-restaurant. It is not clear if the ship ever appeared on Star Trek, but if it did, it's probably from the Dominion war-arc.
71) The Valkyrie was a model shown at the Exhibition. Maybe the ship never appeared on-screen, allthough some believed the Constellation seen in 'Redemption' was the Valkyrie. This can't be confirmed, but it is possible. Since there was a Constellation-class vessel in Redemption, it was either Hathaway or Valkyrie. For obvious reasons I assume it was indeed USS Valkyrie.
72) This is pure speculation. The Voyager-pathfinder model built for presentation purposes was labeled NCC-73602. The DS9TM pictures an 'Intrepid/Constitution-variant' that looks like this ship. Obviously, all ships from the 'Frankenstein-fleet' pictures in the appendix appeared on DS9, obviously 'A Time to Stand' or one of the other war-arc episodes. So if the Pathfinder really appeared on the show, it would be canon. We could argue whether the Pathfinder may have been the Intrepid-class prototype USS Intrepid since it is highly doubtful that there is a second USS Voyager out there.
73) The Yamato's registry in 'Where Silence Has Lease' was NCC-1305-E, this was even spoken dialogue. We might assume that the 71805 was Yamato's real registry while the 1305-E was just a 'secondary' registry to honor the name Yamato. The same that happened to the several Enterprises. Maybe those had 'real' registries, too, but SFC decided just to use the original number. This is only speculation, of course.
74) The existence of this class is still a mystery. The ship appeared several times in the background of DS9, 'Doctor Bashir, I Presume?' and 'Chrysalis', for example. Neither the name nor registry of the ship are known, but Paramount confirmed the vessel type to be the Yeager-class. Either this is a completely new class or there has been an Intrepid-class USS Yeager. Either way, there has to be another ship named 'Yeager' after the Sabre-class ship went out of service or was destroyed. There are also those who think the DS9-Yeager was actually 'Jaeger', named after Alex Jaeger, but since the DS9-staff confirmed the ship was named after Chuck Yeager, this is not the case.
75) Gene Roddenberry suggested the Enterprise-A was in fact the renamed USS Yorktown. If the ship really became the new Enterprise, it is impossible that the same Yorktown was mentioned in 'Flashback' but a newer ship. I assume it was a newer ship, but I think one version of this old story is as good as the other one. Maybe it was, maybe it was not the old ship.
76) The Yorktown mentioned in 'Frame of Mind' could be only a faked ship, like the Yosemite.
77) A cargo container was visible in TWOK on a display with the registry NCC-4000. From FJ's manual, too.
78) The shuttle that brought Stadi and Paris to DS9 was a craft of a starship with that registry. In later scenes, the registry of the shuttle switched between this one, 1701-D and 74656.
79) Allthough the holoship didn't get a final (visible) registry, all sketches and concept drawings bore the registry NX-75115.
80) Data's scoutship bore this registry. Star Trek Armada calls it a 'Venture-class', allthough there's still a starship Venture in service. Other sources give the name 'Talon-class'.
81) The DS9TM gives this strange registry as the original spaceframe registry of the ship that later became the USS Defiant NX-74205.

Further notes:

-  'Federation Starfleet only' means I wont add any ship from 'Enterprise' to this list until it has officially been confirmed that Earth Starfleet is Federation Starfleet (see entry 'USS Dauntless' for example) or we get a definition of what Starfleet is at the time of 2151. Just Enterprise? All earth spaceships? I simply don't know. This may sound a bit nitpicky, but at the moment, it's the best way to solve some problems and inconsistences.
-  Whether the Strarship Enterprise-A was a ship of the 'Enterprise-class' or just an updated 'Constitution-class' doesn't matter. As long as there are no annotations about the state of canon of the controversed 'Enterprise-class', the so-called 'refit' stayes the 'Constitution-class'. Furthermore, we don't know if or how many other ship were refitted like the Enterprise and later the Enterprise-A (or when, in some cases), so I leave this up to everyone who reads this. If the class is given as 'Constitution', this can mean TOS-configuration or Movie-configuration (Enterprise-class / Constitution-Refit).
- The entry (Star Trek Encyclopedia) means this ship has neither been mentioned nor seen in any episode, but the Encyclopedia says there is a whole class of this name, so there must be a prototype vessel. Hence the addition to the chart.
-Whether the Pasteur is a Hope-class ship or an Olympic-class ship is not clear. The dedication plaque of the Pasteur lists the ship as a Hope-class vessel while the Encyclopedia says it is an Olympic-class ship. The 'TNG Companion' says the Hope-class dedication plaque was just an early version and was later replaced. My theory (I know, it's not canon, but there has to be a solution, and since the Encyclopedia doesn't give one...): 'Olympic' is the designation of the class, maybe a cruiser or transport. However, there are 'subtypes' like the Soyuz-class, a member of the Miranda-family. And the Hope-class is just a subclass of the Olympic, a hospital ship. That's the reason why the Encyclopedia lists the Nobel as an Olympic-class vessel and the dedication of the Pasteur says 'Hope-class' instead. Since this whole explanation is not canon, I leave both designations, Hope and Olympic, in the class-chart for every vessel of that type.
- Franz Joseph's Technical Manual is a controverse topic amongst those who say Gene Roddenberry 'de-canonized' it and those who believe that it was the first and maybe only official 'fan-publication' (maybe the only other book to date from the same category is the Starship Spotter, allthough all classes in the book are existing and the information given is just some small extra to give those ships a little more background. So nothing to worry about. :-) ).
There are, however, two major problems: the starships Columbia, Revere, Merrimack and Entente were mentioned in the com-chatter in TMP, with the proper registry from the Star Fleet Technical Manual. Are those ships members of the Cygnus-class (Columbia), Hermes-class (Revere), Bonhomme Richard-class (Merrimack) and Federation-class (Entente) or not? Since no class was given, I tend to say those classes were just inofficial, 'non canon'. I don't like it, but that's a given fact. There are, on the other hand, the Hermes-, Saladin- and Ptolemy-classes, not mentioned by name, but visible on computer-displays in TWOK. Same problem as above, allthough the screens confirmed the name and registry of the prototypes. But until I get a close look at the Technical Manual (meaning I read it and examined every information given in the book) and can decide whether to declare it 'official' or not, I'll leave the ships from TMP classless. So summarizing we have the following:

- Many of the classes (such as Wambundu, Yorkshire, Andromeda for example) are featured in the Encyclopedia. Since every class we saw so far also had a class-ship, I assume it's the same here. So I added all the class ships to the chart, too. Because indirectly, the Encyclopedia confirmed their existence.
- All ships in my chart are 'NCC's' and 'USS's' meaning there are no private/civilian starships such 'SS'/'NAR'. Not yet. :-)
- Several ships from TNG and DS9 later appeared on the casualties-list in DS9's 'In the Pale Moonlight. The wallchart was later pictured in the DS9-Companion. The Following ships were present on that list:

The problem is that some of those ships were already decommissioned/destroyed when the war begun and simply can't be still active. Those units are the the starship Tripoli (disappeared from the junkyard Qualor II in 'Unification') and Sarajevo (Garak said the ship went missing in the Gamma Quadrant). You could invent some fantastic stories to bring them back, but I don't think it's worth the time. Let's just say it was a small error, too small to be noticable on-screen. I'll leave the two ships in the episodes-chart nontheless, allthough I cont them as 'obvious errors'. The wallchart appeared a second time in 'Images in the Sand' (and a third time in 'The Siege of AR-558'). It was the same, allthough several ships had become klingon and romulan vessels. Because of their irrelevance for this shiplist, I wont even try to figure out their names.

All 'controverse' names disappeared, leaving only some ships from the 'In the Pale Moonlight'-list. I'll add those names to the chart, too, even if this appears to be the same list. Let's just say those were just some poor ships, getting under attack three times. ;-)
- The Sovereign's registry has never been confirmed, allthough Mike Okuda suggested using the 75000 because it would be the one used if we ever see the ship and the authors don't plan anything special for the registry.
