Another survey developed for use on Fidelity's Intranet. This survey was used to collect feedback about employee knowledge about community activity work done by the company. I was responsible for all aspects of this survey. This included graphics design, interface design, logic programming, database design and development. Tools used on this project include ASP, SQL, VB Script, JavaScript, Photoshop, HTML

Features include:

• Limited typing by users to eliminate typing errors
• Employee verification by First and Last name using SQL
• Employee verification to remote Sybase System 7 database database
• Only available to sub-set of these employees limited to a specific location
• Form verification using JavaScript to make sure all data is complete
• Collection of all information and storage to a local database
• Graphical reporting feature available to administrators to display stats
• Email reporting feature allowed administrators to recieve results
