A dynamic registration form developed for use on Fidelity's Intranet. This form was used to register employees for various training classes offered by the employee training department. I was responsible for graphics design, interface design, logic programming, database design and development. Tools used on this project include ASP, SQL, VBScript, JavaScript, Photoshop, HTML, DHTML

Features of this application include

• Employee verification
• Form layout changes dynamicaly based upon course chosen
• Class schedules
• Dates and times of class stored in database for update by administrator
• Dates and times of upcoming classes displayed prior to date of class
• If class is closed it will not be displayed on the form
• After submission of form a user verification screen helps verify inputted information
• Printable report is sent to the user and their manager
