This online survey was produced for use on Fidelity's Intranet. I was responsible for database design and development for the administrators of the survey. I was also responsible for the logic programming using ASP, Javascript, and SQL.

Some features of this survey included:

• Employee Verification to a remote employee database using ODBC connection
• Only available to sub-set of these employees limited to a specific location
• Collection of all information and storage to a local database
• Graphical reporting feature available to administrators to display stats
