DENTAL INSURANCE BENEFITS

PORTAGE COUNTY AND STEVENS POINT AFSCME EMPLOYEES

 

 

AFSCME Local 348 sponsors self-pay dental plans that are open for all Portage County and City of Stevens Point AFSCME employees.

 

The employee pays all premiums associated with the dental plan.  These premiums and a small administration fee are deducted from the employee’s paycheck for two payroll periods each month.  A new employee’s initial eligibility period to apply for dental insurance begins on the employee’s date of hire and runs for 31 days.  (For example, an employee hired on October 15 would be eligible to apply for dental insurance from October 15 to November 15.  An employee hired on November 1 would need to apply for dental insurance by December 1.)  If you choose not to apply for the insurance during the initial eligibility period, you may choose to apply during the annual open enrollment period held in March of each year or within 31 days of completing the six month probationary period..  Employees with qualifying events (marriage, new dependent, divorce, loss of coverage) may enroll within 30 days of the occurrence of the qualifying event.

 

Employees who enroll in a dental insurance plan are responsible to notify the President of AFSCME Local 348 (currently Collene Ottum) IN WRITING should they decide to terminate the insurance.  If an employee enrolled in the dental insurance goes on a leave without pay, Local 348 will adjust the insurance deductions accordingly to recover premium amounts paid in for the employee while the employee is on leave.  Should the employee terminate insurance while still owing premiums, Local 348 will seek to recover those amounts from the employee.  Premiums are adjusted each year as necessary based on the rates provided by Guardian Insurance.  Employees may access information through www.guardiananytime.com.

 

AFSCME sponsors these dental insurance plans: Guardian HMO, and Guardian Passive PPO.

 

GUARDIAN HMO:               Monthly premium amounts from April 2008 to March 2009 are : Employee $27.89; Employee and spouse $58.37; Employee and child(ren) $62.20; Employee, spouse, and child(ren) $89.05.  Plan includes youth and adult orthodontics.  This is a managed-care plan and plan participants are limited to the approved area providers.  A list of providers is available with the application or available at www.glic.com.

 

 

GUARDIAN PPO:                Monthly premium amounts from April 2008 to March 2009 are : Employee $38.82; Employee and spouse $82.52; Employee and child(ren) $100.79; Employee, spouse, and child(ren) $142.19.  Plan includes youth orthodontics.  This is a managed-care plan that allows you to choose your own dentist.
