The Order Form
The order form includes
every item offered by choiceZ doing this holiday promotion.
Each product is
listed on the order form by its item number, simply scroll down the
order form list and select the items and quantity you wish to
purchase.
SALES
TAX APPLIES ONLY IN THE STATE OF TX
Your order must be for
$100.00 (before taxes and shipping) or more or the order will not be
processed and an email will be sent to you stating you did not meet
the minimum amount required to process your order.
Our order form is
processed securely by PrestoSell.com and sent to PayPal for
processing.
You
will NOT be asked for credit card information by
choiceZ (that
will be taken only when you complete your order with PayPal). Once
you have chosen your items and completed your shipping information
(your shipping and address information must be the same for credit
card orders) you will receive a confirmation:
You must click the PayPal
logo to continue with the payment process.
If
you do not have a PayPal account you will be prompted to sign-up.
