WHAT IS SELF HARM ? AND WHY IS IT SO COMMAN IN THE YOUNGER PEOPLE OF TODAY What is self-harm? Self-harm refers to the act of self-injury and can take many forms, including overdose of medication, poisoning, hanging and cutting. The terms self-harm, deliberate self harm (DSH), parasuicide and attempted suicide are often used interchangeably and refer to non-fatal suicidal behaviours. What are the risk-factors for self-harm? Self-harm is more common in women with the highest average rates in young (1524) females. The factors that predispose to suicide also increase the risk of self-harm but psychiatric disorders are less commonly identified, especially in the first episode of self-harm. What causes someone to self-harm? The intent of the individual engaging in an act of self-harm broadly falls into three overlapping categories: cessation (stopping conscious experience/death) interruption (to temporarily interrupt conscious experiences, to sleep, not to feel anything for a while) appeal (to request help from others/change their behaviour) (Bancroft 1979). The timing of such an act is often related to recent stressful events especially loss events or criminal proceedings. Will a person who has self-harmed get better? A past history of self-harm is a risk factor for future self-harm or suicide. Thirteen year follow-up of individuals who had been admitted to hospital for self-harm found that up to 68% had no further admissions for self-harm. Thirty two percent of those followed, however, were readmitted with at least one further episode of self-harm over the next 13 years [median number of readmissions = 1 (range 1-137)] (Hall 1998). |
