ADMISSION REQUIREMENTS

[ FRESHMEN ] [ TRANSFEREES ] [ FOREIGN STUDENTS ] [ OLD STUDENTS]

The College of Optometry seeks to admit students who are both academically and personally qualified to complete the four-year professional program and to practice optometry in a highly competent, ethical and responsive manner.

Applicants from all over the Philippines and foreign countries are considered for admission.

The College Admissions Committee sets policies and procedures for admission and recommends to the Dean those persons who are selected for admission. The committee has developed a selection process based on the following criteria :

GENERAL REQUIREMENTS . All 4-year and 5-year collegiate courses are open to anyone who has graduated from a high school recognized by the government. Moreover, they must submit and/or comply with the following :

NEW STUDENTS

FRESHMEN

1.) Pass the CCP entrance examination, except those who have a general average of 85% and above in their fourth year academic subjects.

2.) Uncancelled High School fourth year report card ( Form 138-A )

3.) Four (4) ID pictures ( 1 1/2 x 1 1/2 )

TRANSFEREES

1.) Transfer credentials

2.) Certified copy of grades

3.) Certificate of good moral character from school last attended.

4.) Four (4) ID pictures ( 1 1/2 x 1 1/2 )

FOREIGN STUDENTS

1.) Original copy of Study Permit issued by the Commission of Higher Education (CHED)

2.) Xerox copy ( photocopy ) of the Alien Certificate of Registration (ACR) ; with eligibility for admission issued by the CHED for the current school year.

3.) Xerox copy ( photocopy) of Passport with student visa.

4.) Graduates from foreign secondary schools who may not fully satisfy the specific requirements of certain collegiate programs or courses may be admitted provided the deficiency is corrected during their initial school term or semester.
