Here are the forms!

Here are the required forms  to be filled out and brought to the swim test.  The best way to access these forms is to click on them and let them download.  It may take a while, depending on your modem.  Once they have appeared, simply print them out on your own home printer.  Please do not send these forms to the park... or anywhere else for that matter!  Just fill them out and bring them to the swim test along with your birth certificate.
