STIMULATION BY ACUPUNCTURE OR ACU LASER IN PATIENTS WITH PARTIAL GASTRECTOMY

In order to search for an objective indicator to forecast the analgesic effect, especially for that of viscera, and to investigate the indication of acupuncture anesthesia for partial gastrectomy so as to improve its result, the variation of gastrin - a trace biologic substances in serum-in 16 cases of patients with partial gastrectomy by acupuncture/aculaser anesthesia has been observed since 1980, Blood samples were taken before and after acupuncture/ acu laser stimulation and a radioimmune double antibody separation technique was used to measure the gastrin. Besides, the variations of the volume curve of the digital tip pulse before and after anesthesia were depicted by the physiologic recorder in 7 patients.

1) The variation tendency of gastrin before and after stimulation by acupuncture/aculaser showed: gastrin was raised in 9 cases (P<0.005, P<0,05).

2) To compare the pain ratio of this group with the dosage of aneasthetic used by a combined epidural route (ie. the combined acupuncture group), including that used in the first hour, we found that the pain ratio of the combined group, when gastrin was raised, was lower than that when gastrin was lowered, but the dosage of the epidural anesthetic was a little and the pain ratio of the combined aculaser group, when gastrin was raised, was higher than that when gastrin lowered, but the dosage of the epidural anesthetic was more also. These results indicated that in the case of gastrin raised the pain ratio was high, the dosage used was much and the analgesic effect was not good (the results was not significant in statistics).

3) The comparison of the variation of serum gastrin with the visceral pain ratio showed that the pain ratio was much lower when gastrin lowered, than that when gastrin raised (the results were not significant in statistics either).

4) There were some relations between the variations of the serum gastrin and the volume curve of the pulse on digital tip. Among the 4 cases with gastrin raised, 3 cases showed dilatation or no change in the pulse curve, 1 case with gastrin lowered showed contraction; 2 cases with gastrin not changed showed no or dilatation change. The results indicated that with the acupuncture/aculaser used the sympathetic and vagal activities were interact,, but the condition for them and their mutual relations needed more informations and further study. The variation of serum gastrin before and after acupuncture/aculaser stimulation showed that the visceral analgesia effects in most cases were good when the gastrin was to be lowered, indicating some regular patterns, and it was valuable in reference which was worthy to be further investigated.
