INDIVIDUAL DIFFERENCE BEFORE THE OPERATION OF HYPERTHYROIDISM UNDER ACUPUNCTURE ANAESTHESIA

We are making use of the traditional Chinese diagnosis and treatment method as well as the index physiology and biochemistry for the individual difference before the operation of hyperthyroidism under acupuncture anesthesia.

From 1977 to 1982, we performed 131 cases, among them male: 27 cases, female: 104 cases. Under Chinese medical dialectics: Divided into three groups: 27 cases in the deficiency of "Yang" (Philosophical positivism) (20.6%), 34 cases in the deficiency of "Yin" (Philosophical negativism) (26%), 70 cases in the deficiency of both of them (53.4%).

Acupuncture anesthesia results; As the degree I and the degree II, the deficiency of "Yang" was higher than the deficiency of "Yin" (P<0.05),

Exercise restraint tests and acupuncture anesthesia coincidence rate; the deficiency of "Yang" was higher than the deficiency of "Yin" (P<0,05).

Basic pain threshold and exercise restraint threshold: The deficiency of both was a little lower than the deficiency of "Yang" and the deficiency of "Yin". However, during acupuncture Hegu, Neiguan after the needle guiding for 20 mins, the pain in the deficiency of "Yin" was more endurable than the deficiency of "Yang", as the pain threshold and exercise restraint threshold tests.

Pulse volume wave change: When the pain being stimulated, the amplitude reduced at all, the difference was not clear among the three groups. The deficiency of "Yin" was clearer than the deficiency of "Yang" as the basic line change.

Pulse rate change: The pulse rate of the deficiency of "Yang" and the deficiency of both were a little slower, the pain was being stimulated.

Electric reflection of the skin change: The deficiency of "Yang" and the deficiency of "both" were undulated a little. The deficiency of "Yin" was quite undulated as the pain being stimulated.

The measurement of 17-hydroxysteroid in urine increased in the three groups. The measurement of 17-Ketosteriod in urine: The increase in three groups and the normal were fifty per cent for each. VMA measurement in urine: The deficiency of "Yang" was normal in more cases. The deficiency of "Yin" increased at all. The deficiency of "both" was in fifty per cent.

As before, the function of vagosympathetic nerve system was relatively stable in the deficiency of "Yang". As the pain being stimulated, the pulse rate and the electric reflection of the skin were less movable. The pain threshold and exercise restraint threshold raised much more during acupuncture anesthesia. However, the result of acupuncture anesthesia was better.

The deficiency of "Yin" was in the hyperactivity condition of sympathetic nerve. The pulse rate increased and electric reflection of the skin much more fluctuated as the pain being stimulated. The pain threshold and exercise restraint threshold raised insignificantly during the acupuncture anesthesia. We consider that acupuncture anesthesia is not very well for the patient of deficiency of "Yin".
