To schedule an appointment with me will need to e-mail me with the following information:

Email address
Employer
Job Title
Contact # at work
Cell phone
Best time to call
City and State of residence

*ALL INFORMATION MUST BE COMPLETED!

You will also need to state the desired appointment date and time and the duration of the length of time you would like to spend with me.

*SUPPLYING A REFERENCE BY ANOTHER LADY YOU HAVE SEEN WILL HELP
TREMENDOUSLY IN DETERMINING WHETHER OR NOT WE WILL BE ABLE TO MEET.

I will get back to you as quickly as possible and once we've agreed on a time for meeting, you'll be given instructions regarding a deposit if one is needed.

The time for your appointment begins at the scheduled appointment time. I will be on time and hope that you will show the same curteousy.
