General Guidelines

I. Annual Physical Examination

1. APE will be coordinated with the HR/Personnel Department for the schedule of
the APE with ACCUCARE head Office.
2. APE shall be conducted only to clinics and Hospitals Stated at the letter of
endorsement from ACCUCARE.
3. Patients will present their ACCUCARE ID Card and Endorsement Letter on the
day of the scheduled APE to ACCUCARE Coordinator /staff.
4. For on-site APE (corporate accounts only), coordination will be through the
HR/Personnel Department.
5. The medical results will be sent to the Company’s Human Resources.

II. Out -Patient Availment Procedures

1. Patient will present their Accucare Card to the Hospital Coordinator or the HMO
Office of their preferred hospital /clinic during clinic hours.  They will sign a
Coordinators LOA after the consultation and a referral form if they will be
referred to a specialist other than the Coordinator.
2. In Case patient is required to go through a laboratory test by an Accredited
Physician or Specialist, Issue an Out-patient Diagnostic or Laboratory Form.
3. LOA for all Diagnostic/Procedures must be requested by an Accredited
Physician, Ct-Scan, MRI, Ultrasound, Special Laboratory Test, Out-Patient
Surgery Procedures and all other Request above P500 must be coordinated to
ACCUCARE for Proper approval.
4. For Out-Patient Surgical procedures Filling of Philhealth is required.
