Required Letter to the School Superintendant

In order to comply with M. S. 120.101 as amended, we are writing to advise you of the fact that my husband and I are home educating our son.

_ _ _ _ _ _ _ _ _ _ _ _ _ _age 8 Grade level to be taught 1st grade/possibly beginning 2nd grade (latter half of the year)

Our instructional calendar will have 174 days as required by M. S. 120.101 as amended. The curriculum will include the appropriate subject areas for _ _ _ _ _ _ 's cited in M. S. 120.101 as amended. Our qualification to teach is that we are _ _ _ _ _ _'s parents. Therefore, we will submit a quarterly report card before November 7, 1997. We are including documentation that required subjects are being taught, including a list of texts and a description of what we plan to teach in each subject.

We will be testing our son at or near the conclusion of the instructional year using a nationally norm-referenced standard achievement examination as required by M. S. 120.101 as amended. Please let us know when we can come in and pick up the test as specified in the Student Report for Aids to Non-Public Schools, ED-01650-08.

_ _ _ _ _ _ _ _ has an active I. E. P. on file with the school and we are willing to work with the school to see the services recommended (ie. Speech Therapy, Occupational Therapy, and adaptive Physical Education) be followed throughout the upcoming school year.
