Neuman's Model Applied: part 2
In tertiary prevention, the focus is on the return of the person to wellness. Much of the work Janice would do in an outpatient program would be tertiary prevention because it focuses on returning her to wellness. Tertiary prevention would include exploring anxiety triggers. Her depression would also be explored and she could expect to gain insight about ways to minimize the occurrence of future depressive episodes. This leads to primary prevention which is directed at intervention before symptoms occur.
The final stage of Neuman's model is nursing outcomes, or reassessment. This could be done weekly in some outpatient programs, perhaps in a group or in individual sessions. Janice would be asked about her perceived progress towards her goals and she would receive feedback from the nurse. Perhaps she still has insomnia after Prozac is changed to morning administration. This information leads the nurse back to assessment. Changes in intrapersonal, interpersonal and extrapersonal factors would be noted because change determines the success of the nursing interventions.
This application of Neuman's model was not meant to be all-inclusive, but rather was
to offer you some idea how this nursing model would be implemented in a client situation.                   - Brenda, April 2002
(cont'd from part1).

The nurse would review the effectiveness of previous intervention. In Janice's situation, the nurse would assess her sleep pattern. Janice reveals she takes Prozac at HS. Advising her to take Prozac in the morning, as Prozac frequently causes insomnia, would be secondary prevention. Encouraging Janice to talk about ways to handle her anxiety would also be secondary prevention.
