APPLICATION FOR A MALTESE PASSPORT

Information Sheet for Applicants Residing in Australia Please read carefully before you complete the application form

GENERAL
1. Eligibility: You are eligible for a Maltese passport if you are a citizen of Malta:
2. Fees: cash or bank cheque payable to the Malta High Commission. The fee is payable in advance.
$185 for a 10 year passport for applicants over 16 years of age
$120 for a 5 year passport for applicants aged between 10 to 15 years
$85 for a 2 year passport for applicants aged under 10 years

4. Recommender: Section 8 should be completed and signed by a Maltese or other citizen, who has known the applicant personally for at least two years. The person should be a public officer not below the grade of Principal or analogous grade, a professional person (medical practioner, chemist, etc), a Minister of Religion, Bank Manager or any other person of similar standing.
5. The parents'/tutor's consent (Section 7 of form) has to be completed for all applicants under 18 years of age and the recommender (see paragraph 4 above) must witness both parents'/tutor's signature/s before adding his/her countersignature.
6. ID card no.: provide this information wherever it is requested on the application form but only when an official Identity Card issued in Malta is available. Otherwise, leave blank.
7. Photographs: Together with your application form, make sure to produce two identical copies of a recent (not older than 6 months) coloured photograph of yourself taken on a white background and preferably on non-glossy paper. The size should be not larger than 45mm x 35mm and not smaller than 32mm x 26mm. The applicant shall be looking at the camera with a neutral expression and the mouth closed. Slot machine photographs, textured photographs, group photographs and photographs subject to fading or sensitive to heat, are not acceptable. The recommender is required to endorse thus: Certified true likeness of Mr/Ms .......(name and surname) on the reverse of one copy and add his/her signature.

LOST OR STOLEN PASSPORTS
If your Maltese passport was lost, stolen or destroyed, the event should be reported to the local Police authorities and a copy of such report produced with the completed application. Moreover, a sworn declaration would be required to be made before the Consul General of Malta if the reported missing passport was still valid, for an additional fee of $20.


DOCUMENTS TO BE PRODUCED

Please check whether this applies to you.
If you were (a) born in Malta and went on to acquire Australian citizenship before 10/02/2000; or (b) if you were born outside Malta before 10/02/2000 and automatically qualified for Maltese citizenship through descent from your parents, you need to produce documentary evidence that you have lived outside Malta for an aggregate period of at least six (6) years. Primarily, such evidence should be the current and/or previous passport/s. Other documents are, however, also acceptable such as:
� A residence certificate issued by the Australian Immigration Authorities
� Taxation group certificates
� A letter from present/previous employer/s giving employment history
� A confirmation of school/college attendance

In addition, you will need to produce the documents listed below in accordance with the category you belong to, keeping in mind that only original Public Registry certificates are acceptable.

First Maltese Passport and Renewing a Maltese Passport issued before 01/01/1988 (where applicant is a citizen of Malta by birth in Malta)

For Male Applicants and Unmarried Female Applicants
1. Applicant's birth certificate

3. Applicant's parents' marriage certificate

For Married Female Applicants
The above three certificates plus
1. Applicant's marriage certificate (and the decree nisi where marriage has been dissolved and applicant chooses to go back to using maiden surname)

3. Husband's father's birth certificate
4. Husband's parents' marriage certificate

First Maltese Passport (where applicant was born outside Malta before 21/09/1964)
1. Applicant's full birth certificate
2. Applicant's father's birth certificate
3. Applicant's parents' marriage certificate
4. Applicant's own marriage certificate (but only where applicant is a married female, a divorcee or a widow)

First Maltese Passport (where applicant was born outside Malta on or after 21/09/1964)
1. Applicant's full birth certificate
2. Applicant's father's birth certificate
3. Applicant's parents' marriage certificate
4. Applicant's own marriage certificate (but only where applicant is a married female, a divorcee or a widow)
5. Applicant's paternal grandfather's birth certificate
6. Applicant's paternal grandparents' marriage certificate

First Maltese Passport (where applicant is a citizen of Malta by registration)
1 Applicant's full birth certificate
2 Copy of citizenship certificate
5. Applicant's own marriage certificate (but only where applicant is a married female, a divorcee or a widow)

Renewing a Maltese Passport issued or renewed on or after 01/01/1988 (where applicant has not changed his/her name)

� The Maltese passport, which is due for renewal, is to be produced with the completed application form. Applicants must also present their birth certificate.

� Where applicant is also a citizen of Australia, the Australian passport is required together with the Australian Citizenship Certificate where such citizenship was acquired by naturalisation.

Please note 1. Other documents and/or certificates in addition to those detailed above may be requested to establish without doubt applicant�s claim to Maltese citizenship.
2. It would assist the processing of your application if you were to produce all the required documents when you lodge your application, and if you were to ensure that the details you give on the application form are correct in every respect. If in doubt, it is advisable for applicant to seek clarification at the time an appointment is requested to lodge the application.

EMERGENCY (TEMPORARY) PASSPORT: If a Maltese citizen needs to travel urgently to Malta, a Temporary Document will be issued for a fee of A$35, valid only for the minimum period required for completion of the journey to Malta. On arrival in Malta, an application will need to be lodged for a regular passport at the Malta Passport Office.
