DECLARATION FOR PENSION
Act of May 1, 1920

State of Kentucky, County of Clay:
On this 1st day of Jan. 1923, before me, the undersigned, personally appeared Frelin Collins, who makes the following declaration as an application for pension under the provisions of the act of Congress approved May 1, 1920.

That he is 85 years of age; that he was born May 9 - 1840, at Big Creek, Ky.

That he is the identical Frelin Collins, who enlisted Jan. 14, 1865, at Irvine, Ky, under the name of Frelin Collins, in Co. A. 56 inft., and was honorably discharged July 14, 1865, at Irvine, Ky, having served the United States, in the Civil War.

That he also served in the Three Fork Portalian [sic], Union Army.

That otherwise than herein stated he was not employed in the United States Military or Naval service.

That his personal description at time of first enlistment was as follows: Height 5 feet 9 inches; complexion Dark; color of eyes Gray; color of hair Black; that his occupaton was Farming.

That since leaving the service he heas resided at Big Creek Ky and his occupation has been Farming.

That he requires the regular personal aid and attendance of another person on account of the following disabilities:
part deaf Rheumatism
that he has required such regular aid and attendance since 10 years ago.
That he has not applied for pension under Original No. -; that he is not a pensioner under Certificate No. -
