TREATMENT OF EJACULATORY DEFICIENCY BY ACUPUNCTURE

According to the view of traditional Chinese medicine, this dysfunction is caused by the disorder of the "liver" and "kidney" function. Normally "the Ren meridian takes the charge of the uterus and foetus", therefore it has a close relationship with reproductive function- According to the theory of western medicine, this dysfunction is due to overinhibition on the ejaculatory center by the cerebral cortex, or the weakness of the ejaculatory center. Therefore, Qugu and Yinlian on the "liver", "kidney" and Ren meridian, relating to the ejaculalory center through the nervous segment, are taken. As the cortical representative areas of sensation of the hallux and reproductive organ are both closely located in the paracentral lobule of the cerebral cortex, and in addition, the cortical representative area of sexual organs, the dominants sex-reproductive center in the limbic system and its subcortical centers are closely related in morphology and function, Dadun of hallux can also be taken for treatment.

Owing to the mechanism of the readjustment of the function of ejaculatory center in-the spinal cord and sex-reproductive centre on the cerebral cortex, the acupuncture treatment is effective in ejaculatory deficiency.

Clinical Data

There were 82 males treated with ages ranging from 25 to 39. Their reproductive organs examined by conventional urologic methods were normal. A history of nocturnal emission was usually revealed. They were diagnosed to have functional ejaculatory deficiency and did not respond to various therapies before acupuncture treatment.

Method of Acupuncture Therapy

1. Selection of points: Points to be taken according to the traditional view. Points with sensation of referred pain. Only needling on such points could bring about the needling reaction and gave strong effect.

(1) The main points selected: Qugu (needling), Yinlian (needling) and Dadun (Moxibustion).

(2) The coordinate points selected: Ciliao was added in patients accompanied by impotence, Zusanli in the weak patients, and Baihui, Neiguan in insomniac patients.

2. Manipulations: The depth of the needling for Qugu ended with the emergence of the sensation of an electric shock towards the root of urethra. The depth of the needling for Yinlian ended with the local emergence of sensation of soreness, distention and heaviness. While the patients felt strong enough about the needling sensation and the acupuncturist experienced a weighty, light and stagnant sensation in his needle-twisting fingers, the hand technique of twisting acupuncture needle needed the method of "Pingbu Pingxie" (mild reinforcing and attenuating) or the method of the light and fast twisting. The needle was twisted for about one minute and remained for five minutes. When the patients have weak needing sensation and the acupuncturist had a light, loose and smooth feeling in his fingers, the manipulation of "Fei" or slow twisting was used. The needle was twisted for about two minutes and remains for ten minutes. The needle was slightly winded when it was taken out. Dadun was in moxibustion with the twig of the Chinese moxa,

3. Therapeutic Regimen: The needling was done once every two or three days, and one course of treatment took ten times. A second course might be given after a week, if the patient did not respond to the first one. During the course of treatment, moxibustion of Dadun was necessary once a day. Sexual intercourse should be prohibited during the first course of treatment. Generally the patient would be cured after one or two courses of treatment. Those patients who have strong needling sensation might be cured within the first course.

Criteria of Therapeutic Efficacy

1. Good: Ejaculation is normal during sexual intercourse,

2. Failure: Ejaculatory deficiency persists after five courses of treatment.

Results

Among the 82 patients treated, 70 were good. The treatment failed in 12 cases. The effective rate was 85.36%.
