Please enroll my Child/ward or myself for the Top rank Tennis Academy I have read and accept the
policies listed on this form. I Authorize Top Rank Tennis/Coach V to Charge my Credit card through
Paypal below, in full for all programs selected. I understand that there are no refunds or credits given
once I have been charged. Terms And conditions of enrolment in Top rank Tennis or Coach V's Programs
and Packages: Full payment on "due date" specified, or later than day # 1. Installments available only to
families with multiple members enrolled in program/s: equal payments over number of months equal to
the number of enrollees - i.e: 2 months for 2 members, 3months for 3 members etc. Enrollment is for
entire semester/package - norefunds/credits for withdrawals, unless for medical condition verified by
doctor's note. If a payment s 30 days past its due date, a charge in the full amount will be posted to this
credit card account. Request for changes in day/time of enrollment must be in writing - Coach V or Top
Rank Tennis will try to accommodate such request, but does not guarantee it. Make-ups are not guaranteed
- however, Top rank Tennis & Coach V will try to accommodate make-ups to the best of its ability. Make-
ups must be completed before the end of the given semester/package and cannot be carried over. In
circumstances beyond Top Rank Tennis & Coach V's Control (i.e Inclement weather, local/national crisis
or other emergency) will not provide make-ups, refunds or credits. If your regularly scheduled
Professional is unavailable, Your lesson/sesion will not be cancelled or postponed, but will be taught by
another Professional as directed by the Director of the department.
Waiver: I understand and acknowledge the risks of injury that are
inherent in any program involving physical activity, and as the
enroollee/Parent or guardian of the child registered herein, I hereby
waive and release any and all rights and claims for damages I may
have against, Coach V, Top Rank Tennis academy, Program
Directors, instructors or employees and any sponsors and their
respective representatives for any loss of property or for any and all
injuries in connection with participation in this program. I have no
objection to photographs taken with the program being used in
advertising or other media efforts. I do hereby give my consent to
medical care, emergency or otherwise, inclusive of any necessary
transportation, in the event of injury to myself/ my child or ward.
Program Requirements & Player responsibilities
- High Performance Team, Endorsement Team & Touring
Team you must have a USTA Card Prior to Tryout Date,
Futures Team must have their USTA Card Prior to First
Tournament
- Commitment, Respect, Patience, Listening are core values
of the Program. Please adopt these values
- Rain days/ make-ups may extend weeks of the program
- Poor attitudes Racket Breaking and complaining about
Conditioning or other Team events will not be tolerated.
- Two USTA Tournaments Minimum must be played for Junior
Teams
- Program Rates: $500 Flat Rate Must be Paid in Full If you or
your child is accepted into TRTA. Financing is available for
those who qualify. Prices are subjected to change.
- Players Must Bring Own Water Jug, lunch, Water and
Bathrooms located near courts.
- Please state any illnesses, Prior to tryouts
- No Parent "Coaches" During Practices it may distract other
players, coaches or other parents.
- Please Keep Tennis Facilities Clean
