This document is for MIS feedback. Feedback forms on the Intranet provide companies with a great channel for their employees to submit feedback. Employees can send their comments, problems, and suggestions to MIS through a form.

If employees are having problems with their workstation, for example, this is a great way to collect information about their machine, what problems it has, etc. The form can be created so that this feedback goes to the appropriate person. For example, a drop-down menu for the type of feedback (eg; comments, bug reports, suggestions, questions) can help guide this information to the right people.
