| PAPERWORK DUE TO ANNA MARIA BEFORE THE FIRST DAY OF PRACTICE These five (5) forms must be given to the athletic director before athletes are allowed in the water. Athletes must also turn in all uniforms from other sports in which they've participated. Physical This is the only form the school will accept. You need a new physical every other year. Parental Permission You need a new form each year. Athlete's Contract You need a new contract each year. Uniform Contract You need a new contract each year for each sport. Emergency Medical History You need a new form each year for each sport. The clearance list is posted in the gym outside Anna Maria's door. |
