| Your Primary Care Trust
Birmingham East and North PCT [abbreviated to BEN]employs District Nurses, Health Visitor, and Nursery Nurse who work within the practice. The PCT can be contacted at Suite 20, Waterlinks House, Richards Street, Aston, Birmingham B7 4AA. Their Telephone number is 0121 333 4113. Disabled Access At Small Heath Medical Practice Wheelchair access to the building is via a ramp near the front entrance. Patient Services are provided at ground floor level. There is also access to a low-level counter at the reception desk. Disabled WC is provided. If access proves difficult to any of our disabled patients, we would be happy to consider any suggestions. Patient Rights and Responsibility Our patients have the right to be treated with respect and without prejudice. We would request that patients attend punctually for their appointments and cancel appointments if they are unable to attend. Repeated failure to keep appointments not only causes in convenience, but also denies other patient the opportunity to see the doctor. Staff have the right to be treated with respect and should not be subjected to verbal or non-verbal violence. If patients demonstrate, unacceptable behaviour they may be removed form our practice list. Data Protection Act This practice holds both written and electronic medical records for patients, neither of which is disclosed to a third party without the patient�s written consent |

| Confidentiality Code of Conduct
This code of practice is applicable to all staff employed by Small Heath Medical Practice and to all other attached staff who access confidential information about individuals. | How to Register as a Patient
New patient are welcome from the within our defined practice area.You can check if your residence is within our practice area by contacting our reception desk. When registering the receptionist will firstly ask you to complete an application form then proceed with your application if acceptable. You will then be encouraged to book a new patient check with the practice nurse. This is a free check up and will give you opportunity to discuss your medical history in more detail with the practice nurse. If you are outside our practice area then consult the following website.
