PHOTO & VIDEO PERMISSION FORM

	During each school year, photographs and video will be/have been taken 
of the students.  These pictures are taken during normal school day activities 
during class, recesses and even on field trips.  All originals are kept in the 
classroom for students and parents to view at their convenience and leisure.  
Copies (when available) are given to students at Mr. Sabre’s expense.  I am 
providing concrete memories for the students of their childhood and time in 
Mr. Sabre’s class.  Often times, some photographs or video will be made 
available on class related web sites for students and parents to view, remember 
and enjoy.  The sites are also intended to provide a means for students and 
parents to get in contact with Mr. Sabre or each other (if they have lost 
touch with one another).

This form requests permission from the parent(s) or guardian(s) to allow use 
of the photographs or video to be posted on any educational web page linked 
to www.MrSabre.com.
