High School / Elementary
New students/Transferees:
1. Results of the Entrance Exam
2. Report Card (F-137)
3. Certificate of Good Moral Character
4. Birth Certificate
5. Two (2) 1x1 ID Picture
6. Recommendation letter from the Principal, Class Adviser or Guidance Counselor
Old Students:
1. Report Card (F-137)
2. ID picture (1x1) 3 pcs.
3. Short brown envelope (1pc)
For Alien / Foreign Students:
1. Letter of application stating the intended course;
2. Official Transcript of Records and Diploma;
3. Certificate of Good Moral Character from the Principal;
4. Affidavit of support of the person supporting the applicant's study;
5. Financial statement from the bank regarding the capabilities of the person funding the study of the applicant;
6. Five (5) passport size pictures;
7. Five copies of personal history statement from duly accomplished;
8. Photocopy of Passport;
