Health Behavior Inventory

Directions:  Read each statement carefully.  If the statement describes you, click on the box to indicate a positive action that promotes health literacy, maintains and improves health, prevents disease, and reduces health-related risk behaviors.  When finished, print this out to track future changes in health behaviors.


Mental and Emotional Health
I cope with stress in healthful ways.

Communicable and Chronic Diseases
I use sunscreen to protect myself from the sun.
I have information about my family's history of disease.

Injury Prevention and Safety
I have a list of emergency telephone numbers.
I wear a safety restraint when riding in a car.

Consumer and Community Health
I have a budget that I follow.
I participate in school clubs and community activities that promote health and safety.

Environmental Health
I recycle.
I listen to music at safe levels.
